Rank One Forms
Mathis ISD uses RankOne Sport to help manage all UIL athletic forms. Once you have created your account you will be able to complete and submit all forms through your account. All forms will also be signed through e-signature in your account.
- Please ensure you select the correct school and grade for the upcoming school year
- Please use ONLY school registered name and school ID# on all forms (No nicknames)
- To sign document click inside the signature box and hold mouse down, this will allow you to create an "Electronic Signature." If you make a mistake and need to start over click on the refresh icon next to the signature box.
- Once you have filled out all the information on each page you will have the opportunity to print the document
- You will receive a confirmation email after each form has been submitted and once the document has been reviewed by the staff

Students wanting to participate in Athletics for the 2026-2027 school year are required to submit all UIL forms before being allowed to try-out, practice, or compete in any UIL athletic sport(s). Students in every grade, 7th-12th, are required to submit a physical form along with all other required UIL forms for the 2026-2027 school year
